APPLICATION PROCESS

• Login to QIIB Mobile or Internet Banking
• Select eRequest > New Request > HIMYAN Card, Input the required information > Submit

Card will be issued and sent to the selected delivery address within 3 working days.

Customer can login to QIIB Mobile and Internet Banking to request for the Replacement / Reissue of HIMYAN Card

Complete the HIMYAN Card Dispute Form and Submit to the Branch
