 WHAT IS THE APPLICATION PROCESS?

• You should have a Corporate Account open with QIIB or open a new Account with QIIB
• You will need to sign “Corporate Cash Deposit Card Application Form”
• You will need to sign “Corporate Cash Deposit Card Terms and Conditions”
• Complete the 'Source of Funds Declaration'
• The application must be submitted to the QIIB Corporate Branch
